NASW-NYS: So, your client has been diagnosed: Engaging the Hospice Conversation
With an increasing focus on length of stay, readmission, and reimbursement, the goal of this training is to empower the voice of the social worker to identify patients whose goals of care should be changed from aggressive treatment to comfort/hospice focused measures. Data shows that many Americans prefer to die at home however, very few are granted that opportunity. Moreover, hospice has been known to save hospital systems dollars by preventing readmissions and lowering length of stay. This training will help social workers to engages providers, patients, and families in the difficult conversations related to Palliative Care and Hospice as well as transition patients and families on to these programs.
