Ideation to Implementation: Collaborative Care and the Current Payment System

Historically, social workers in healthcare settings have primarily provided psychosocial assessments, discharge planning, and supportive counseling (Fraher et al., 2022). While essential, these functions alone are insufficient to meet the complex needs of today’s diverse patient populations. Healthcare reform initiatives, including value-based care, Medicaid transformation, and integrated care models, prioritize reducing avoidable hospitalizations, enhancing care coordination, and addressing health-related social needs (Centers for Medicare & Medicaid Services [CMS], 2023). These priorities align with core social work competencies; however, organizational barriers, role ambiguity, and limited training often hinder their full integration (Morse et al., 2023). Following this course, participants will be able to:
- Examine the evolving role of social workers in care delivery.
- Develop skill related cross-training and workforce development.
- Integrate and apply new knowledge of the Triple P Approach: Partnership, Practice, Payers.
