As death due to other chronic diseases (such as heart disease) declines, more people live to an age where the risk for dementia is highest. It is essential that people are aware of the planning needed to ensure they receive the kind of care they want and deserve in advance of a dementia diagnosis. Compassion & Choices (C&C) is the nation's oldest, largest, and most active nonprofit working to improve care, expand options, and empower everyone to chart their end-of-life journey. C&C launched a 2019 program with the release of a pair of free online tools: the Dementia Values & Priorities Tool and the Dementia Decoder. C&C speakers will review the options available to those planning for a dementia diagnosis as well as demonstrating these two new tools. This highly interactive session will leave participants with concrete steps they can take to support their families and the individuals they serve, those planning for a dementia diagnosis, and two new tools to help document care preferences.
