    COVID-19 has brought a renewed focus to the health disparities our most vulnerable populations have been struggling with for years. In Illinois, a shift in priorities has occurred as constricted access to social services impacts the way hospitals and health systems connect patients to resources provided by community-based organizations, FQHCs, and agencies across the state. This session will discuss what clinical social workers have implemented to meet the growing needs of patients and their families. The challenges social workers face will be identified as well as what has worked to close the loop on referrals. Additionally, results of a recent Chicago-area social worker interview and survey will be shared; it will discuss the impact of COVID-19 on patients and how needs have changed with housing, food insecurities, unemployment, etc.
