    This audio program details the speakers 10 year journey creating an integrated health care model and demonstrates how the program has evolved over time and works in action at Compass Medical PC.  The speaker explores the value of this model to patient, provider, health center and health care community.  She demonstrates the processes developed at Compass including use of EMR (electronic medical records), training manuals, physician training and community resource guides to enhance the services provided to their patients.  Finally, the speaker offers insight related to why medically complex patients have better results when an integrated model is offered. 

    Learning Objectives:
