When identifying factors for success for people seeking to achieve and maintain long term recovery from substance use disorder one item that stands out is the need for individualized program management. During this presentation we will outline several strategies that we have found successful in our efforts to provide individualized recovery planning in a transitional living environment. Among those strategies are, empowering people to take control of their recovery, motivation through personal goal setting, and holding people accountable to their own recovery plan. We believe these methods are effective, and we are excited to share them!
