    Ethical Considerations in Clinical Documentation: Client, Clinician and Payor Perspectives

    Presenters: Susan Jasko, LICSW; Tanja Philhower, MSW, LGSW; and Nancy Houlton, MSSW, LICSW

    The same clinical documents - Assessment, Treatment Plan and Progress Notes - serve different purposes for the client, clinician, and payor. For example, the Assessment tells the client what the provider's informed thinking is on the presenting problem and underlying causes, clinicians develop the treatment plan based on the problems identified in the Assessment, while payors look at the Assessment and resulting Treatment Plan's congruity in addressing the issues identified. This panel will discuss how these formal documents - Diagnostic Assessment, Treatment Plan and Progress notes - can ethically serve these diverse functions. Participants will be invited to join the discussion at the end of the presentation.


    About the Presenters
    Susan Jasko, LICSW is a psychotherapist and supervisor at Hamm Clinic, where she coordinates documentation services for clients seeking assistance with immigration issues. Ms. Jasko has thirty years of clinical experience with children, adults and families. Prior to the Hamm Clinic she worked for nine years at the Center for Victims of Torture, providing diagnostic evaluation and treatment services to clients, and clinical supervision of staff. Ms. Jasko is a Community Faculty Member at the University of Minnesota School of Social Work, where she teaches Family Therapy.

    Tanja Philhower, MSW, LGSW worked for the University of Florida Police Department as a victim advocate for eight years before moving to the UF Dean of Students Office and spending over eight years as the director of the Care Team, the university's student case management office.  Ms. Philhower has worked in non-profit child abuse prevention services, homelessness services, and hospital-based care management.  Ms. Philhower currently serves as a veterinary social worker with the University of Minnesota's Veterinary Medical Center where she is receiving supervision for clinical licensure in Minnesota.  Ms. Philhower has worked as an MSW field instructor both for Florida State University and schools of social in the Twin Cities area.

    Nancy Houlton, MSSW, LICSW (she/her/hers) is a licensed independent clinical social worker mental health professional with over 40 years of clinical experience with adults, children and families experiencing severe mental illness, substance abuse and/or chronic physical conditions.  Ms. Houlton provides consulting services to Behavioral Health and Managed Care Organizations as well as DHS ranging from implementation of new clinical services to behavioral health policy changes.  Ms. Houlton’s previous roles included Behavioral Health Director at UCare, Ramsey County Mental Health Center Director and Treatment Director at Mental Health.
