To help close this gap between the care wanted vs. what is received, grassroots efforts are being made on state and community levels. Conversation of Your Life is one such program, focusing on engaging NJ communities in conversations around advance care planning (ACP). Social Workers are a crucial component in helping to ensure New Jerseyans’ wishes are respected. This workshop will provide strategies for normalizing this rich conversation in your practice in all settings.
